and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees, in that employees were
exposed to struck-by hazards by ejected
parts:
On July 15, 2003, employees were exposed to the hazards of being struck by
a 1,100 pound, unbalanced part
from the Giddings & Lewis CNC Vertical Boring Mill, S/N 513-59-79, company
No. V10, during post
milling cleaning operations.  While in manual mode, the table, with the
part mounted, was spun at a high rate
of speed, in an effort to remove chips, cutting fluids, coolants, and
water by centrifugal force, causing the part
to be ejected from the chuck clamps.  Feasible and acceptable methods to
eliminate this hazard would be, but
not limited to: A) secure the selection of the table speed manual mode of
operation
by either key-switch,
access code or an equally secure means capable of being supervised;  B)
establish a written safe cleaning
procedure, properly train the operator in the use of the procedure, the
hazards involved and ensure the
operation of the machine is properly supervised, as established in ANSI
B11.23-2002, Safety Requirements for
Machining Centers and Automatic, Numerically Controlled Milling, Drilling
and Boring Machines.
