Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
Employees working with, and in the vicinity of, the East Crane ( Whiting
Trambeam Underhung
Crane ) in Building 1A were exposed to the hazard of being struck by loads
which were handled
with the crane, or by loads falling from the crane, in that employees
operating the crane had not
received training in it's safe operation, including the operator
requirement to test functional
controls and safety devices, such as the crane's upper hoist limit switch,
on a daily basis.
Further, the crane was not being inspected on a frequent ( daily to
monthly ) basis and was in
service with unsafe conditions including a pendant control box with
unlabeled function buttons,
and a damaged pendant control cord. Among others, one feasible and
acceptable
method to
correct this hazard is to implement a crane inspection program including
frequent ( daily to
monthly ) inspections in accordance with ANSI B30.11, the American
National Standard for
Monorail Systems and Underhung Cranes, and to train / qualify operators of
the crane, also in
accordance with that standard.e
