N.J.A.C. 12:110-5.8(b): The employer did not report every fatality or
serious injury
orally and in writing, within 8 hours to the Commissioner of Labor.  The
reporting
may be by telephone or electronic data transmittal.

LOC: The In-patient Hospitalization Of John Aufiero, Worker, Which
Occurred On September 9, 2002.
The report of the fatality or serious injury was not received at the NJ
Department
of Labor, Office of Public Employees Occupational Safety & Health as
required.
COMMENT: The reporting may be by telephone or electronic data transmittal
and the written report required by
the above shall mean the execution of the employers First Report of
Accidental Injury or Occupational Disease.
ABATEMENT DATE: Immediately
PENALTY:
NOTE: The employer is required to submit to the Office of Public Employees
Occupational Safety & Health a
written procedure providing that in all such future instances a report
will be made immediately to the NJ
Department of Labor, Office of Public Employees Occupational Safety &
Health using the Hot Line # 1-800-624-1644 and/or the Facsimile #
609-292-3749.
