Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by and/or crushing hazards due to falling loads
and/or lift parts.
a) Main Line Ford Service Garage - The surface mounted Challenger Model
37,000
Automatic Lift in "B" bay was used improperly in that only two (2) of the
four lift arms
were used to elevate a 2.5 ton Ford F-250 truck causing it to slide off
and injure an
employee. This was observed on October 4, 2002.
b) Main Line Ford Service Garage - The Rotunda lifts in Service Bays A,D,F
and G (i.e.
Hunter and Rotary respectively) were not marked with their load rating
capacity and
warning labels. This was observed on October 4, 2002.
NOTE:  Among other methods, one feasible and acceptable  abatement method
to
correct this hazard is to: 1) Implement a periodic inspection and
maintenance
schedule to be performed as required by the Automotive Lift Institute
Standard for
Automotive Lifts-ANSI/ALI ALOIM-2000, Safety Requirements for Operation,
Inspection and Maintenance; 2) Provide employee/operator training on hazard
recognition and safety procedures as well as enforcement of the
procedures; 3)
Document employee operator training and inspection and maintenance
performed;
4) Post operating instructions contained in the "Automotive Lift Safety
Tips Poster".
