North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling loads: a) facility -- the upper limit switches were not operable on the two 20 ton reel cranes on #11 and #12 machines, dry end south, and the two 10 ton overhead cranes in the #13 and #14 machine room south and north. In the alternative: 1910.179(n)(4)(i): At the beginning of each operator's shift, the upper limit switch of each hoist was not tried out under no load. Among other methods, one feasible and acceptable method to correct this hazard is to instruct the operators to try each upper limit switch, under no load, at the beginning of each shift, and to repair inoperable upper limit switches immediately.4)(i):

Among other methods, one feasible and acceptable method to correct this hazard is to replace the rotted flooring.e

Among other methods, one feasible and acceptable method to correct this hazard is to instruct the equipment operators to report back-up alarms which are not working and repair the back-up alarms immediately.
