North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards while working from high lift forklifts at heights of up to 60 feet without using fall protection. a) site, narrow aisle vehicle #248, Raymond, model # 89F20SRT, SN899000281, on/or about May 7, 2002, an employee was not attached to the lanyard connected to the NAV while working from the platform at a height of approximately 50 feet above the concrete floor. Reference: Manufacturer's operating procedures. Among other methods, one feasible and acceptable means of abatement method to correct this hazard would be to train employees to wear fall protection and enforce these procedures.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to fall hazards while using damaged fall protection equipment. a) site, narrow isle vehicle #248, retractable lanyard attached to vehicle had a knot tied in it approximately 6' from hook end. b)site, shock absorbing lanyards had been subjected to impact loads and not removed from service. c)site, employees were not trained to recognize damaged fall protection equipment Reference: ANSI Z359.1-1992, paragraph 6-1.4, 7-2.1, 7-3 Among other methods, one feasible and acceptable means of abatement method to correct this hazard would be to train employees to inspect fall protection equipment on a regular basis, removing damaged equipment from service.t
