Carolina:  The employer did not furnish each of his employees conditions
of employment and a
place of employment free from recognized hazards that were causing or
likely to cause death or
serious physical harm to employees in that employees were exposed to
overhead hazards:
a)production area- where employees used a Lodestar 2 ton hoist installed as
an underhung crane to lift fork lift batteries onto a table so that they
could
be charged and/or serviced.
production area- where the underhung crane had not been tested to confirm
the load rating of the system and the employer had not been conducting
frequent and regular inspections of the crane and monorail systems.
Abatement note:
One feasible and acceptable method, among others, is to perform the
following:
a)  have the underhung crane load rated by a Certified Professional
Engineer
to
ensure that the structure can withstand the load capacity and
b)  implement frequent and regular inspections of the crane and monorail
systems.
