Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
(a)  Employee(s) operating the Robbins and Myers pendant controlled hoist
were exposed to
shifting or falling load(s) due to (1) improper reeving/crossed cable of
the 4-part line resulting
in uneven and rough assent of the load block and any attached load(s) and
(2) cable that was
crimped and bent and which did not seat properly in the grooves of the
hoist drum.
Among other methods, feasible and acceptable means for abating this
condition include
instituting repairs to bring the hoist into compliance with the
manufacturer's specifications and
with the Overhead Hoists (Underhung) standard, ANSI/ASME B30.16, for the
safe operation
and use of underhung overhead hoists, including applicable requirements
for
thorough, periodic
inspections, implementing procedures to regularly inspect the hoist and to
ensure the safe
operation and performance of components at regular intervals, and to train
employees in the
recognition of potential hazards associated with hoists.ted
