North Carolina General Statute 95-129(1) of the Occupational Safety
and Health Act of North Carolina:  The employer did not furnish
employment and a place of employment which were free from
recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were exposed
to:
a)fall from an elevation,in that on or about May 9, 2002
the following condition occurred:employees were working
from a two point suspension scaffold(swing scaffold) in
which the hooks were not resting on a surface capable of
supporting 4 times the load imposed on them,the
connection was not evaluated to confirm that the support
surface was capable of supporting the load,u-bolt clips
were used on the fiber rope and the u-bolt was not placed
over the dead end of the rope.
Among other methods,one feasible and acceptable abatement
method to correct this hazard is to provide support that as
been assured and verified by a competent person that
the support surface will support 4 times the intended
load,and to inspect the rope prior to use to assure the
u-bolt clip are properly installed with the u-bolt on the
dead end and the saddle on the live end.

following condition occurred:employees accessed a two
point suspension scaffold(swing scaffold) utilizing an
aerial lift and were not wearing a belt and harness and
had not been properly trained in their use.
Among other methods one feasible and acceptable method to
correct this hazard is to require the use of a belt and
harness when being elevated by an aerial lift and that
employees have been properly instructed and trained in there
use.e
