21 VSA Title 23, Chapter 3, Sub-Chapter 5, Section 223(a): Section 5(a)(1)
of
the
Occupational Safety and Health Act of 1970:  The employer did not furnish
employment and
a place of employment which were free from recognized hazards that were
causing or likely
to cause death or serious physical harm to employees, in that employees
were exposed to a
crushing hazard due to inadequate maintenance and lack of an electric
brake on the hangar
door and of an inadequate alarm system during operation of the hangar
door.
Location: Burlington Tech Center, Burlington International Airport, "Alert
Hangar
Building", Pod 2
Violation: The employer did not furnish each of the employees employment
and a place of
employment which are free from recognized hazards in that employees were
exposed to the
hangar door that did not operate properly during the opening or closing of
the east hangar
door, thus exposing employees to a hazard of being caught between the door
and the wall;
and employees were exposed to hazards due to the lack of adequate alarms
during the door(s)
operation.
Among others, feasible and acceptable methods to correct the hazards
include, but not
limited to properly balancing the door in accordance with the Maintenance
and Operational
Manual for the Butler Alert Hangars, and/or installing an electric braking
system. An electric
