exposed to being struck by a brass nozzle attached to a whipping
pressurized fire hose.
CENTRAL ARTERY TUNNEL, C17AA, at 101+51 Standpipes:
On or about August 21, 2002, an employee was struck by a 10 pound 31 inch
long brass nozzle attached to a pressurized fire hose that he was unable to
manually control during a flow test.
One feasible method, among others, of abating this hazard would be to
provide employees an
engineering control capable of and designed for the purpose of securely
holding fire hose nozzles
during flow test procedures.
