Section 5(a)(1) of the occupational safety and health act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and or crushed by the overturning of Jib cranes. (a) Location - Fabrication Area: The foundations for a 1-Ton Jib crane and (2) 2-Ton Jib cranes were not constructed in accordance with the crane manufacturer's recommendations. Feasible abatement methods include but are not limited to the following: 1. Consult a qualified structural engineer to determine that your structures are adequate to support the loads generated by anchor bolt force, overturing moment, or axial load of your cranes. 2. Construct support foundations for the cranes according to the manufacturer's installation manual, in accordance with the foundation and footing reference charts.

Section 5(a)(1) of the occupational safety and health act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to hazards associated with not being familiar with operating and safety manuals specific to the model aerial platform being used : (a) Location - Machine shop: Employer does not ensure employees are properly trained in the safe operation of aerial platform lifts as in accordance to operators manual. Feasible abatement methods include but are not limited to the following: 1. Consult Marklift Industries to facilitate training for employees assigned to operate and provide proof of training. 2. Ensure qualified personnel train employees to understand the manufacturer's operating instruction and user's safety rules.
