Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards associated with attaching verticle life lines to the swing scaffold the employees were working from. Location: 140 Clarendon Street, Boston MA. An employee was seriously injured when the swing scaffold wirerope failed and it fell to the ground. The employees life line was tied off to the scaffolding guardrail system which could have broken the life line resulting in fatal injuries. Among other methods, one feasible and acceptable method would be to train employees not to tie the ends of their verticle life lines to the scaffolding guardrail system.il
