A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that the employer did not ensure the proper use of wire rope securing devices (i.e. Crosby clamps) in accordance with the manufacturers recommendation: (a) 150 North 18th Avenue Phoenix, AZ 85007: The employer did not ensure the proper use of wire rope clips (ie Crosby clamps) in that the "U" bolt was not in contact with the dead end of the wire rope.
