40.1-51.1(a) of the Code of Virginia:  The employer, Valmar, Inc., did not
furnish a place of employment which was
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that
employees are operating a Mitsubishi propane forklift, which was located
in the Kitchen Production Area, which
exposed them to:
Contusions and/or fractures by not having an operable seatbelt on the
forklift.  One side was broken completely off.
One feasible and acceptable abatement method, among other methods, to
correct this hazard is to install a new seatbelt
on the forklift such as that called for by ASME B56.1-1993, American
Society of Mechanical Engineers for Safety
Standards for Low Lift and High Lift Trucks.ds
