Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an amputation hazard:
(a)On or about 8/26/02 in the Secondary (Machine) Shop; a Multipress
hydrualic
press, Model #WUA-2 serial #6973 (company ID #D192), the two-hand trip
sensor buttons were not set up to require the release and repressurization
of both
buttons in order to activate the ram.   The ram could be activated by
keeping one
button depressed and activating it by pressing the 2nd button.  The ANSI
B11.2-
1982 section 3.3.4, requires that two hand controls be:  (i) arranged, by
design,
construction and/or separation so that concurrent application by both
hands is
required on each cycle of the press.  (ii)  designed to require continuous
pressure
from both hands while there is a hazard at the point of operation.  and
(iii) shall
incorporate an antirepeat feature.
