Title 40.1-51.1.(A), Code of Virginia: The employer did not furnish employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: In the front of the building, approximately six 3 foot high 3/4 diameter concrete form pins, being used in concrete curb forming operations, had mushroomed heads exposing employees to the hazard being struck by flying pieces of metal. " ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD, AMONG OTHERS, TO CORRECT THESE HAZARDS IS TO IMPLEMENT AN INSPECTION AND MAINTENANCE PROGRAM TO ENSURE THAT THE HEADS ARE GROUND OR CUT OFF BEFORE THEY BECOME MUSHROOMED".
