40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: OEM Plug Machining Department - A Dayton bench grinder with a 6 inch General Industrial Diamond Co., Inc. vitrified bond abrasive wheel was used to grind non ferrous (brass) lock components. The improper use of this type of wheel could cause the wheel to fail and debris to be ejected from the grinder.
