Code of Virginia -  40.1-51.1(a):   The employer, Virginia Department of
Transportation, did not furnish a place of
employment which was free from recognized hazards that were causing or
likely to cause death or serious physical
harm to employees in that employees were operating a 2-ton ATD
hydraulic/mobile hoist; a 2-ton Budget hand chain
operated hoist, and a 1 ton Philadelphia hand chain operated hoist in the
main shop, that had not been inspected in
intervals required by the American National Standards Institute (ANSI)
B30.16-1987.
At the Charlottesville Residency shops, a  2 ton ATD- hydraulic hoist; a 1
ton Philadelphia  & a 2 ton Budget hand
chain operated hoists had not been inspected, monthly or annually, and
records maintained of those inspections.
Specifically frequent (monthly) visual inspections of all controls and
operating mechanisms safety devices, hooks,
chains, and chain stretch must be conducted; periodic (annual) documented
inspections of loose bolts, excessive wear
of chain, chain stretch, hook, supporting structure, and end connections
of load chains must be conducted; and  a
written, dated, and signed inspection report and record to indicate
required inspection must been completed for items
inspected  inspected during an annual inspection.
Among other methods, one feasible and acceptable method to correct this
hazards id to conduct monthly inspection
such as that call for by ANSI B30.16- 1987, American national Standard
Safety Code for Hoists.
