Code of Virginia -  40.1-51.1(a)  The employer, Greene County Vehicle
Maintenance Facility, did not furnish a
place of employment which was free from recognized hazards that were
causing or likely to cause death or serious
physical harm to employees in that employees were operating  hydraulic
hoists in the shop, which had not been
inspected as called for by American National Standards Institute (ANSI) in
consensus standard B30.16-1987.
At the Vehicle Maintenance shop the hoists and not had frequent (monthly)
visual inspection of all controls and
operating mechanisms safety devices, hooks, chains, and chain stretch.
Periodic (annual) documented inspections of loose bolts, excessive wear of
chain, chain stretch, hook, supporting
structure, and end connections of load chains had not been conducted.
Written, dated, and signed inspection reports and record to indicate
required inspection had not been completed for
items inspected  inspected during an annual inspection.
The orange 1/2  to 8 ton long ram mobile boom hoist was  in use on the day
of the inspection, with a distorted safety
latch on its hook.
Among other methods, one feasible and acceptable method to correct this
hazards id to conduct  inspections call for by
ANSI B30.16- 1987, American national Standard Safety Code for Hoists.
