40.1-51.1(a) of the Code of Virginia:  The employer, McQuay International,
had not furnished a place of employment
which is free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees
in that employees were operating a jib crane with attached electric hoist,
without having received training and passing a
practical operating examination, as called for by American National
Standards Institute (ANSI) B30.16 (Safety Code
for Electric Powered Hoists).#266, which is
In Area C18 at the Goff Washer, employees operate a 1000 pound Harrington
Electric Hoist, ID #266, which is
mounted on a jib crane, when moving parts to the parts washer.  Employees
are  exposed to back injuries when they
must manually move the hoisted parts.  All appointed operators have not
been trained nor have they passed a practical
operating examination before operating the electric hoist.
One feasible and acceptable method to correct this hazard, among other
methods, is to provide training and a practical
operating examination for hoist operators such as that called for by ANSI
B30.16, American National Standard Safety
Code for Electric Powered Hoists.
