The employer did not furnish to each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to; Ref: ANSI B30.10-1.4.6(a)(5): Inoperative safety latch Note:Among other methods, one feasible and acceptable abatement method to correct this hazard is to replace the safety latch. (a)Air Drill area -- safety latch on a monorail crane was damaged.

(a)Shipping -- In house made below the hook lifting devices had not been load tested.
