Nevada Revised Statute 618.375(1):  The employer did not furnish
employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death
or serious physical harm to employees in that:
The employer did not ensure that employees were protected from falling at
the following
location:
a.North side of Building #7, Snorkelift Aerial Work Platform,
Model #TB-37, Serial Number #9712580497, five employees rode
the bucket of the aerial lift to gain access to the roof without being
tied-off.  These employees were exposed to a potential fall of
approximately 25 feet to the ground below.
Reference:  ANSI/SIA A92.5-1992.  Boom-Supported Elevating Work Platforms.
Section 8.8:  During operation.  The aerial platform shall be used
in accordance with this standard.
Section 8.8(g):  Ensure all personnel in the platform are wearing
fall protection devices and other safety gear as required at all
times.
Reference:  Operator's Manual, Snorkelift Model TB 37.
General Operating Cautions.  Item 18.  All personnel in platform
MUST wear a safety belt.  Secure safety belt to platform rail,
NEVER to an adjacent object or structure.
One method of abatement is to train/retrain employees on the proper wear
and anchorage of
personal fall protection equipment in accordance with ANSI A92.5 and the
operator's manual
for each lift before operating the lift.ting
