Section 88.4 - Code of Iowa (2001) 88.4: The Employer did not furnish to each of his employees employment and a place of employment which is free from recognized hazards that are causing or are likely to cause death or serious physical harm. Employees were exposed to: (a)Hines Tube Bender Area - In a hazardous situation involving an unguarded machine and the emergency release button on the foot pedal not working, the employer failed to address these issues and to train employees properly. An employee amputated a thumb and two (2) fingers while operating the Hines Tube Bender, serial number-0625 2000.7, model number-300NC. Among other methods, one feasible and acceptable abatement method to correct this hazard is to do a hazard assessment and make repairs on this machine. Also adopt an effective training program and re-train all employees that operate this machine. Referenced by ANSI B11.12-1996. This condition was noted on September 17, 2002.mber
