North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free from
recognized hazards that were causing or likely to cause death or serious
physical harm to employees in that employees
were exposed to:
a)facility - safety latch on the Coffing model 622, serial number PA3, 2
ton underhung manual chain
hoist was not inspected and repaired when found defective. Reference ANSI
B30.16, section 16-2.1.2,
paragraph c-6.
Among other methods, one feasible and acceptable method to correct this
hazard is to establish a
program of inspections of hoists in the facility to detect defective parts
and safety latches or devices on
hoist hooks and replace broken parts as needed.
b)facility - load chain on a Coffing 2 ton underhung electric chain hoist,
model LHH, serial number
0005, was modified by doubling and fastening together the chain with a
bolt, washer and nut, which
reduced the load bearing capacity of the load chain. Reference ANSI
B30.16, section 16.1.2.1,
paragraph b.
Among other methods, one feasible and acceptable method to correct this
hazard
is to shorten the load
chain by removing the chain links and re-attaching the chain with factory
approved link connector of
the same load rating.
c)facility - metal lifting devices attached to the hoists for lifting the
rolls were not marked with the
manufacturers load ratings. Reference ANSI B 30.20, section 1.2.2.
Among other methods, the metal lifting devices purchased and manufactured
by the employer should be
marked with the load rating and designed to match the load rating of the
hoist system.
d)facility - periodic and documented inspections of all hoist systems to
include the connecting frame,
trolleys, hoist with components and below the hook lifting devices were
not conducted by a designated
person. Reference ANSI B30.16, section 16-2.1, paragraph b 2.
Among other methods, one feasible and acceptable method to correct this
hazard is to designate a
qualified to conduct the periodic inspections and document the results of
such
inspections.
