Section 5(a)(1):  The employer did not furnish employment and a place of
employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical
harm to employees in that employees were exposed to the hazard of being
caught in a moving
conveyor belt:
a)Sorting conveyor belt area.
The sorting conveyor belt area was not provided with an emergency stop
device
for all operators, thereby exposing employees to being caught and dragged
by the
conveyor belt and then being crushed by the force of the belt dragging the
employee into the chassis, on or about 9/19/02.
Abatement Note: Among other methods, a feasible method to eliminate or
materially reduce this
hazard is to comply with ASME/ANSI B20.1-1996, section 5.11.2(c) and
provide permanently
installed pull cords or similar stop controllers at points where employees
are normally stationed.
NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION
FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY
OF $1,000.00 AS PER 29 CFR 1903.19.
