from recognized hazards that were causing or likely to cause death or

a)facility - being struck or caught in between the lift and the ground in
the event employees fall
or get thrown off.  The employees operating the 30,000 lb all terrain
Clark forklifts were
exposed to being caught in, between/crushed from not using the installed
seat belts.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to train/certify and require employees to wear seat belts installed by
manufacturer of Clark
forklifts.er
