death or serious physical harm to employees, in that employees were
exposed to:
a)being struck or caught in between the lift and ground in the event
employees
fall or get thrown off.  The employee operating the SKYTRAK Forklift, model
7038, was exposed to being caught/crushed from not using the installed seat
belt.
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to train/certify and inspect employees on wearing seatbelts
installed by
manufacturer of the SKYTRAK forklift.rer
