Section 5(a)(1) of the Occupational and Health act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious harm to employees in that employees were exposed to crushing hazards: Bay one: At the time of the inspection the safety dogs on the swing arm lift # 5691 were not engaging. One feasible and useful method to correct the hazard, among others, is to: 1.Have a competent person/company thoroughly inspect the lift and perform needed maintenance and repairs. 2.Develop, implement and document a program of regular inspections per the lift owners service manuals.
