Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of getting caught in inrunning nips on moving conveyors: Or, in the alternative: 29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by inrunning nips and moving/rotating parts: a)On or about 2/28/03 and 6/5/03, throughout the facility, numerous inrunning nips were not guarded and emergency stops were not within reach of employees. On 2/28/03, an employee was severely injured while working at the side of the primary conveyor of the ERS line. His right arm was caught in the unguarded nips of the moving conveyor and was not able to reach the emergency stop causing his arm to be amputated at the shoulder. Among others, one feasible means of abating the hazard includes: Installation of guarding at inrunning nips along the conveyors and especially at the prime feed conveyors which meet the requirements of ANSI/ASME B20.1-1987, Safety Standard for Conveyors and Related Equipment, and to install or move existing emergency stops so that they are within reach of employees working near the inrunning nips. CASINGS, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.212(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 304468473, CITATION NUMBER 1, ITEM NUMBER 1, ISSUED ON 8/8/02 WITH RESPECT TO A WORKPLACE LOCATED AT ROUTE 9W, CATSKILL, NEW YORK. ABATEMENT DOCUMENTATION AND CERTIFICATION REQUIRED
