Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that:
a)145 Cordell Rd, Colonie, NY - on or about December 5, 2003 - an employee
was
exposed to a hazard of falling from the bucket of the front end loader
while riding
this bucket and being struck or run over by the Caterpillar front end
bucket
loader.  Methods to correct this hazard include: 1) Develop and implement a
comprehensive safety training program for employees on the use of heavy
equipment. 2) Follow the manufacturers recommendations in the Safety
Section
of the owners manual, and not allow employees to ride on the machine unless
additional seats with all of the proper safety equipment has been
installed.
