A.R.S. 23-403.A.:  The employer did not furnish to each of his employees
employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to his employees in that employees were loading and
unloading storage racks that were not
anchored to the floor, and that they did not have plaques stating the
maximum permissable load per bay in
clear legible print posted in conspicuous locations.  Both the ANSI
MH16.1-1974, titled Specifications for the
Design, Testing, Utilization of Industrial Steel Storage Racks, and
Interlake's user manual for storage rack
products, mandate that each column footplate be anchored to the floor, and
the load ratings for the storage
racks be posted in clear view.
(a) 840 Rodeo Drive, Prescott, AZ: The employer did not ensure that each
footplate of the 1 5/8" x 3"
storage rack columns was anchored to the floor.
