Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to a fall hazard at the 14 foot level when installing
standing seam metal roofing
from the platform of a boom supported elevating work platform.
(a)Front Exterior - An employee was observed working on standing seam metal
roofing approximately fourteen (14) feet above the ground, from the
platform of
a JLG manlift.  The employee had the door to the lift open and was not
wearing
a personal fall arrest system and tied off to the lift, on or about
February 5, 2004.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
includes using a personal fall arrest system, with the lanyard attached to
the boom or basket of
the personnel lift, as outlined in the American National Standards
Institute
safety standard for
Boom-Supported Elevating Work Platforms (ANSI/SIA A92.5-1992)

29 CFR 1926.453(b)(2)(v):  Employee body harness and lanyard assembly was
not attached to
the boom or basket when working from an aerial lift.
