Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees employment and a place of employment
which were free from
recognized hazards that were causing or likely to cause death or serious
physical harm to
employees in that employees were exposed to falls and struck-by hazards:
(a) Worksite, Southwest Corner of the building - On June 14, 2002,
employees
modified an extendible boom aerial lift platform, adding wooden forks for
use as
an equipment lift platform. This exposed employees to injuries from falls
and
struck-by hazards.
One feasible means of abatement would be to comply with the American
National
Standards Institute (ANSI) A92.5-1992, Standard for Boom-Supported
Elevating
Work Platforms and ensure modifications of an aerial platform are made only
with prior written permission of the manufacturer.
