recognized hazards that were causing or likely to cause death or serious

(a) Highway 27 North Main, Cedartown, GA, on or about April 2, 2003 -
During the
operation of the Hitachi EX230LC-5, hand signals were not used among the
pipe layers
and the operator of the Hitachi EX230LC-5 for direction of work.  Hazard
of being
struck by the bucket.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
(1) Provide training to the workers on how to recognize and use hand
signals and/or
other forms of communication to aide in directing excavation operation.
