Minn. Stat. 182.653 subd. 2:  The employer did not furnish to each
employee, conditions of
employment and a place of employment free from recognized hazards which
caused or were
likely to cause death or serious injury to employees:
Employees were exposed to fall hazards approximately 24 feet while working
from a
personnel basket attached to the forks of a rough terrain forklift that
was telescoped to
the house roof.  The basket was not equipped with an adequate guardrail
system.
Employees were not wearing any personal fall arrest equipment.  Location:
