Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: On March 12, 2004 and at times prior thereto, employees were exposed to electrical hazards and other hazards associated with the operation of the Otis Three Stop Electric Freight Elevator (Serial Number 115390, Sales Number 214198) due to the fact that the elevator and its components have not been inspected or maintained in accordance with ASME A17.3-2002, Safety Code for Existing Elevators and Escalators. Items addressed in the Safety Code affecting employee safety are, but not are limited to: a. The door accessing the machine room was not locked and was easily accessible to unauthorized personnel. b.Hoistway doors are not equipped with vision panels or interlocks which prevent operation of the car when the door is left open. c.Car entrances are not equipped with doors or gates. d.The upper and lower Normal Terminal Stopping Devices (1st and 3rd stop limit switches) do not slow down or stop the car automatically, at or near the tip and bottom terminal landings. ***THIS ITEM REQUIRES DOCUMENTATION OF ABATEMENT***
