Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of amputation and fracture: (a) Press Brake Area- On the Amada RG-50, Serial Number 509617, hydraulic power press brake the means of fixing the selection of modes between "off," "inch," and "single stroke" were not being supervised by the employer. (b) Press Brake Area- On the Accupress #29, Serial Number 2989, hydraulic power press brake the means of fixing the selection of modes between "off", "inch", and "single stroke" were not being supervised by the employer. (c) Press Brake Area- On the Amada RG-50, Serial Number 509915, hydraulic power press brake the means of fixing the selection of modes between "off", "inch", and "single stroke" were not being supervised by the employer. (d) Press Brake Area- On the Amada RG-50, Serial Number 504790, hydraulic power press brake the means of fixing the selection of modes between "off", "inch", and "single stroke" were not being supervised by the employer. Among other methods, one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform to the American National Standard Institute (ANSI) B11.3- 2002, sections 6.11.1 and 6.11.3.4 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazard of lacerations: (a) Tool Room- The Doall Welder vertical band saw, Serial Number 5112337, was not equipped with a magnetic restart or similar device as prescribed by the American National Standards for Metal Cutting Saws, ANSI B11.10-1990, section 5.2.3.4, that in the event of an interruption in the electrical supply, would prevent the saw from restarting automatically when power is restored. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of amputations and fractures: (a) Press Brake Area - The Cincinnati Shaper Power Press Brake, Serial Number 24512, had foot pedals used by employees while bending metal. The foot pedal was not adequately covered to prevent unintended activation. Employees were exposed to amputation hazards from the point of operation. Among other methods one feasible and acceptable method to correct this hazard is to include, but not be limited to the following: (1) Conform the American National Standard Institute B11.3-2002, section 6.11.3.1 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d)
