Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that stacker crane
operators were exposed to injury caused by uncontrolled
descent/accelerated movement of the
crane carriage and operator cab:
(a) Burris Logistics, 111 Reese Avenue, Harrington, DE 19952, High Density
REM -
Storage/Retrieval System, Stacker Cranes Numbers 1, 2, 3 and 4 - the over
speed
sensing devices and associated free fall stop mechanism (explosive bolts)
as provided on
the stacker cranes when manufactured/installed were deactivated by the
employer and
were found to be not functional.  On or about 1/5/04.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable method to correct this
hazard is:
1. Install an over speed sensing device and automatic braking system
consistent with
ANSI B30.13-1977, Section 13-1.10.7, and
2. Install a supplemental braking system that affords the stacker crane
operators the
ability to manually stop the movement of the carriage/cab under conditions
that do not
exceed over speed criteria.
