Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the following condition(s): a) For the period of time up to and including 10 Jun 03, the employer did not have installed an auxillary means of escape installed to the derrickman's working platform to provide a ready and convenient means of escape in accordance with American Petroleum Institute (API) Standard RP 54, third edition, paragraph 6.10.1 which states: "On all land rigs, the derrick or mast shall have an auxillary means of escape installed prior to personnel working in the derrick." This exposed employees working in the derrick to the hazards and injuries related to being trapped in/on the derrick during an event taking place at the well head. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to assure that the auxillary means of escape, also know as the "Geronimo LIne", is attached at rig up and maintained during all operations.
