Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of being struck by a 15 inch wide by
24 inch long hatch
cover.
a)  Employees were allowed to pressurize (via air) a saddle tank on Vessel
Hull #1729
without the use of an automatic pressure relief device set to manufacturer
specifications.
The saddle tank was over pressurized, causing the hatch cover to blow off
and strike an
employee. Employees were exposed to a struck by hazard.
Among other methods, one feasible and acceptable method to correct this
hazard
is to utilize a Manometer designed to measure the pressure of liquids
and/or
gases.
