Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employmnent and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards: (a) 729 Lake St., Wilmette, IL.: Employees were using a wooden job made ladder without the filler blocks inserted between the rungs to support the cleats. The employees were exposed to a fall hazard when they used the ladder. Among other methods, one feasible and acceptable method to correct this hazard is to include, but not limited to the following: (1) Conform to the American National Standard Institute (ANSI) A14.4-1992, section 4.5.2 Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
