employee(s) were using
a wooden job made ladder without the filler blocks inserted between the
rungs to support the
cleats.  The employee(s) were exposed to a fall hazard when they used the
ladder.
Among other methods, one feasible and acceptable method to correct this
hazard is to include,
but not limited to the following:
(1) Conform to the American National Standard Institute (ANSI) A14.4-1992,
section 4.5.2.
Abatement documentation is required for this item in accordance with the
requirements of
29 CFR 1903.19(d).
