that were causing or likely to cause death or serious phyical harm to

(a) The hazard of falling approximately 20 feet to ground level below
while cleaning off
debris from the scrap incline conveyor from a safety platform being raised
by a Hyster
forklift that was not secured to the lifting carriage or forks and was
missing guardrails on or
about March 23, 2003 and at times prior thereto.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
is to follow the guidelines set forth in ASME B56.1-2000 Safety Standard
for Low Lift and
High Lift Trucks and install guardrails and secure the safety platform to
the forklift.
