Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
On or about 9/23/02 employees using, or working in the vicinity of, the
freight elevator in the
facility at 9 Huffman Ave. were exposed to falling into the open elevator
hoistway shaft at times
when the hoistway doors were open and the elevator car was not in place at
the floor landing.
Employees were exposed to falling into the hoistway shaft in that the
elevator was not provided
with hoistway door interlocks to prevent the doors from being opened from
the landing side
when the elevator car was not in place at the landing.
Among others, one feasible and acceptable method to correct this hazard
would be, in
accordance with ANSI/ASME A17.1 The American National Standard Safety Code
for
Elevators
Dumbwaiters Escalators and Moving Walks, to install hoistway door
interlocks to prevent
opening the hoistway doors from the landing side unless the elevator car
is at the landing.
