Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free form
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees were
exposed to the hazard of being struck by a horizontal member supporting
rugs.
a.  On or about September 12, 2003, employees were exposed to the hazards
of being struck
by a dislodged and swinging horizontal rug support member during the
process of raising the
rug support member.  The support member connection failed and released the
support
member that swing downward and struck an employee working the floor.
One feasible and acceptable abatement method among others, to correct this
hazard to
provide regular periodic inspections signed by a qualified person to
determine if the hoist,
wire rope, clips, pulleys and other apparatus are safe to be used in
accordance with the
American Society of Mechanical Engineers (ASME) B30.7-2001, section 7.2.1
- 7.4.2 and
wire rope users manual by the Wire Rope Technical Board Safety Bulletin.

DOCUMENTATION OF ABATEMENT FOR THIS ITEM.
