Section 5(a)(1) of the Occupational Safety and Health Act of 1970
Job site: 2502 Pinehurst Cir., Papillion, NE 68133 -- The employer did not
furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that the employees
were exposed to the hazard of falls from an elevated platform.
(1) Employees performing elevated work on a platform raised by the forks
of a powered
industrial truck are exposed to fall hazards due to the lack of an
adequate guardrail system on
the platform.
(2) Employees performing elevated work on an elevated work platform that
was not firmly
attached to the lifting carriage and/or the forks of the forklift.
(3) Employees performing work on an elevated platform with neither an
operator in the
cab/observing nearby nor the means to shut off the truck power from the
platform
level.
Among other methods, feasible and acceptable methods of abatement are:
1. Provide a standard personnel lift with all the required safety features.
2. Follow the safety procedures provided for in ANSI B56.1-2000 for
elevating work
platforms and operator restraint systems.
3. Verify that the platform is constructed and used in accordance with
ANSI B56.1-2000
which states that a guardrail shall have a height of no less than 36
inches or more than 42
inches.
4. Require employees to wear safety harnesses and lanyards of the proper
type and length
while working from unguarded elevated locations.
5. Secure the platform to the forklift carriage or forks with a safety
chain.
6. Provide for an operator to be in the cab when the platform is at its
working height, or to
be observing work progress from the near vicinity.
