A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were loading and unloading storage racks that were not anchored to the floor. The ANSI MH16.1-1974, titled Specifications for the Design, testing, and Utilization of Industrial Steel Storage Racks mandates that each column footplate be anchored to the floor: (a) 16060 North 81st Street Scottsdale, AZ 85260: The employer did not ensure that each footplate of the storage rack columns was anchored to the floor.ed
