A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were loading and unloading a storage rack that was not anchored to the floor. The ANSI MH 16.1-1974, titled Specifications for the Design, testing, and Utilization of Industrial Steel Storage Racks mandates that each column footplate be anchored to the floor. (a) Shop area, Northwest corner: The employer did not ensure that each footplate of the storage rack columns was anchored to the floor.ed

A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to a storage rack that did not have a plaque stating the maximum permissible load per bay in clear legible print posted in a conspicuous location, in accordance with ANSI MH 16.1-1974, titled Specification for the Design, Testing, and Utilization of Industrial Steel Storage Racks. (a) Shop area, Northwest corner: The employer did not post a legible printed plaque in an easily visible location stating the maximum load capacity per bay.
