Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of being struck by equipment in that:
a)  On or about 03/24/03 an employee was jacking an aircraft.  The
employee was using
a jack which had a load cell installed on the tip of the tail jack.  In
order to install this load cell
on the tip of the jack, parts/ adapter plates were locally manufactured.
The locally manufactured
parts and the load cell used in this configuration were not approved by
the Jack or Aircraft
Manufacturer and could lead to instability of the aircraft while being
jacked. An employee was
injured when the aircraft shifted.
Among other methods, one feasible and acceptable method to correct the
hazard would
be to purchase a load cell from the jack manufacturer or equivalent that
is
designed to be
installed and used on the particular model jack involved.r
